- Q. What address do I send claim appeals to?

The Appeal Form must be filled out completely and attached to supporting documentation. If you do not have an Appeal Form, please contact Customer Service at 850.478.2326 or 800.664.0146. - Q. Once a patient is referred to a specialist (by a primary care physician), who then is responsible for any referrals made by that specialist? Is it the primary care physician or the specialist?
- A. Once the patient has a valid referral to a specialist, it is the specialist's responsibility to obtain authorizations for any services required. The only exception would be a specialist referring to another specialist. At that point, the specialist would be responsible for contacting the primary care physician who would then obtain the referral.
- Q. Does Health First Network have a timely filing limit? If so, what is it?
- A. All claims must be received within 90 days from the date of service.
