• Managed Care Treating Provider Registration Form

  To view or print a copy of the brief 2-page Managed Care Treating Provider Registration form which can be used by Providers who are not already enrolled with the Florida Medicaid Program, click here. Providers may submit this form to obtain a Florida Medicaid Provider ID Number to meet the state provider credentialing requirement and the federal requirement for submission of encounter data.
